Watch this trace. The agent thinks: I need the weather. It calls get underscore weather for Tokyo. The weather service returns an error, five oh three. Then the agent answers: it is fifteen degrees and raining, wear a raincoat. Confident, specific, and invented. Tap the line where it went wrong. The answer. After an error the model had no observation, so it filled the gap from memory and presented it as fact. A tool call that fails must change what happens next: retry, try another tool, or tell the user. Never let the model talk past a failed call as if it had succeeded.
